The Good Faith Estimate (GFE) is an estimate provided by a healthcare provider regarding out-of-network costs. It applies to clients who are self-pay, uninsured, and/or not using a superbill for out-of-network benefits. A GFE shows reasonably expected costs associated with healthcare items or services in relation to a diagnosis and reason for seeking care.
If you schedule a health care item or service at least 3 business days in advance, you should be provided with a GFE in writing within 1 business day after scheduling. If you schedule a health care item or service at least 10 business days in advance, you should be provided with a GFE in writing within 3 business days after scheduling. If you ask any health care provider or facility for a GFE before you schedule an item or service, you should be provided with a GFE in writing within 3 business days after you ask.
If you receive a bill that is at least $400 more for any provider or facility than your GFE from that provider or facility, you can dispute the bill.
A GFE shows reasonably expected costs based on information provided at the time it was created. Unknown costs could arise during the course of treatment, and these unknown costs are not included in the GFE. You could be charged more if additional costs arise.
Treatment frequency is determined by continual assessment of progress on goals, as assessed by both the counselor and client.
A new GFE will be issued should needs or frequency changes during the course of treatment. Length of treatment and frequency of appointments is determined by client circumstances, client finances, and counselor's availability.
